Report of Occurrence v.2

In a step to make it easier for teams and meet administration to file a Report of Occurrence at practice or at a meet, USA Swimming Member Services has implemented an online Report of Occurrence Form. A Report of Occurrence needs to be filed for any accident occurring during a team practice or during a swim meet.  When submitting online, a copy of ROO must be sent to SCS electronically or a copy must be sent with the meet report.  Notification of serious injury (head strikes) should be sent (e-mail or fax) to SCS immediately.
