High School OBSERVED Sanction App - League Champs

Please find the 2021 Observed Sanction App for High School under FORMS, upper right corner of this website.

For this year, due to Covid changing meet dates, the sanction is due a minimum of SEVEN days
prior to the meet. The two USA Swimming officials assigned to the meet must be named on the form with the 
Meet Admin running the meet, responsible for submitting the results.

Mail the hard copy with the $25.00 fee to the SCS office, or you may email it to Nicole and request to charge your team ACH account to expedite the process.

This is not the High School rcoach or representatives responsibility.  If club coaches wish the swimmers times to be accepted from these meets, they need to submit the application.
